Patient Safety Concerns and Complaints

Your safety and your trust matter to us. If something about your care worries you, or you believe we have not met our own standards, we want to know. You do not need to wait for a bad outcome to speak up. You can raise a concern at any time, in any way that works for you.

If you are in immediate danger or having a medical emergency, call 911. If you are in a mental health crisis or thinking about harming yourself, call or text 988.

Tell us first

You can share a concern or complaint with us by phone, email, in writing, or in person. You can ask any staff member to pass it on, and you can ask to speak with a member of our leadership team.

Arise Health Clinic 10705 Charter Dr, Suite 430 Columbia, MD 21044 Phone: 301-259-3574 Email: info@arisehealthclinic.com

When you contact us, it helps to share what happened, when it happened, and who was involved. You can share as much or as little as you are comfortable with. We will listen, look into it, and let you know what we found and what we are doing about it. You can ask us to keep your concern private to the extent the law allows.

You can go to outside organizations at any time

You do not have to speak with us first. You may contact any of the organizations below whether or not you have raised your concern with us.

The Joint Commission. Arise Health Clinic is accredited by The Joint Commission. You can report a patient safety event or concern about a Joint Commission accredited organization.

Phone: 1-800-994-6610

Mail: Office of Quality and Patient Safety, Joint Commission, One Renaissance Boulevard, Oakbrook Terrace, Illinois 60181

The Joint Commission does not accept faxed or emailed reports. It also cannot accept copies of medical records, photos or billing invoices.

Maryland health professional licensing boards. For a concern about an individual nurse practitioner, physician, counselor, therapist or social worker, you can contact that person's Maryland licensing board. The Maryland Department of Health lists each board on its File a Complaint page.

U.S. Department of Health and Human Services, Office for Civil Rights. For concerns about discrimination, see our Nondiscrimination and Language Assistance Notices.

We will not punish you for speaking up

No one will be treated differently, lose access to care, or face retaliation because they raised a concern or filed a complaint, with us or with any outside organization.
