Cancellation Policy
Cancellation Policy for Arise Health Clinic - Effective 10/01/2023 1. Appointment Cancellation: We understand that unforeseen circumstances may arise, and you may need to cancel or reschedule your appointment. To ensure the efficient operation of our clinic and accommodate other patients, we kindly request that you provide at least 24 hours' notice for appointment cancellations. 2. Cancellation Fees: Appointments canceled with less than 24 hours' notice or missed appointments without prior notification may be subject to a cancellation fee of $75. 3. How to Cancel or Reschedule: To cancel or reschedule an appointment, please contact Arise Health Clinic at 301-259-3574 during regular business hours or info@arisehealthclinic.com. You may also leave a voicemail if you're calling after hours. 4. Exceptions: Exceptions may be made for documented emergencies or extenuating circumstances on a case-by-case basis. Our clinic staff will work with you to reschedule you. 5. No-Show Appointments: Repeated instances of no-show appointments may result in a discharge from the facility. 6. Billing and Payment: Any applicable cancellation fees will be billed to you and are due before you next appointment. A payment link will be sent to you 7. Rescheduling: We encourage you to reschedule missed or canceled appointments promptly to continue your treatment effectively.
