Clients, family members, caregivers, referral partners, and other members of the public are encouraged to share concerns, complaints, or grievances regarding the care, treatment, services, safety, or experience provided by our organization.

If you have a concern or grievance, we encourage you to contact Arise Health Clinic so that we have an opportunity to review and address your concerns.
Concerns may be submitted verbally or in writing.

Arise Health Clinic is accredited by The Joint Commission.
Anyone who has concerns about the quality or safety of care provided by Arise Health Clinic may also report those concerns directly to The Joint Commission.
You are not required to submit a complaint to Arise Health Clinic before contacting The Joint Commission.
